A HOSPICE Program can provide...

FAYETTE COUNTY HEALTH DEPARTMENT HOSPICE
offers the family unit choices and control over what happens during the dying and bereavement phase of their lives.  Its main objective is to promote acceptance and emotional healing so that the final opportunity for human communion can truly be experienced.

A Hospice Program can provide...

  1. Supportive, palliative care in the home for as long as the patient/family can manage.
  2. Symptom control encompassing the physical, emotional, social and spiritual areas.
  3. A care team including home health nurses, social worker, clergy, and supportive care volunteer working closely with patient's physician.
  4. Bereavement follow-up for up to one year after death.

when all curative measure for illness have ceased and comfort is the primary concern.

when time is measured in days instead of years.

when we need to accept the fact that change is the only constant in our lives.

Guidelines for admission require...

  1. the patient to live in the Fayette County Health Department service area.
  2. a documented diagnosis from patient's physician of a terminal illness with six months or less to live.
  3. a documented acceptance from patient and family to cease all curative measures and receive only supportive care and pain control.
  4. a responsible caregiver to be  in the home or easily accessible 24 hours a day.

To become a Fayette County Health Department Hospice Volunteer...

VOLUNTEERS are the heart and soul of hospice services.  The main qualifications are a sympathetic approach to fellow human beings, sensitivity to the needs of others and a sincere desire to help maintain the dignity and self-worth of a dying patient.  These human qualities can benefit a hospice program through direct patient care, assisting in the office, marketing the concept to local clubs and organizations, or helping with fund raisers.  If you would like more information about becoming a volunteer or to be notified of the next Hospice Volunteer Training Program contact:
