Home Health Care

Fayette County Health Department Home Health staff has expanded and moved to 120 N 5th Street in Vandalia, IL to better serve you. The Fayette County Health Department Home Health Care Program provides Skilled Nursing (Visiting Nurses), Medical Social Services, Physical Therapy, Speech Therapy, Occupational Therapy and Certified Nursing Assistant Services. These services are provided in the home of patients who are primarily confined to their home under physician's care, but who are not in need of the level of care provided in the hospital or a nursing home. This cost effective service allows the patient to receive his/her individualized care at home, while recovering in familiar surroundings.

Services include:

Visiting Nurses provide skilled nursing care under a plan developed by patient, doctor and Fayette County Health Department  nurse.

Wound and Ostomy Specialist provides consultation and treatment protocols.

Certified Home Health Aides provide assistance with personal care and exercise programs.

Physical Therapy, Occupational and Speech Therapists provide rehabilitation for patients following an individualized plan.

Licensed Clinical Social Worker provides support, counseling, and referrals for appropriate service to patients and families involved in Fayette County Health Department Home Health Care.

Pediatric Nursing-Provides Skilled nursing care for infants and children.

Bringing Health Care Home

Each patient has an individualized plan of care to acquire the best medical, physical, and psychosocial outcomes. The plan of care includes your physician, one-on-one care, family/caregiver assistance, patient education, disease intervention, and promotes independency to your healthcare needs.

The goal of home health is to reduce recurrent hospitalizations and reduce complications. Patient and family compliance enhances the care we give.

How Is Home Health Care Paid For?

Medicare, Medicaid, or Insurance often covers the cost of services. For patients not covered by these insurance programs their bill, if they wish, can be adjusted to the clients ability to pay. Services are available to Fayette County residents regardless of ability to pay.
